In order to guarantee the accuracy and integrity of signing up for this program, we do not accept applications directly online. However, to request an application to be mailed to you, please fill in the information below.

We will mail you an enrollment form with a pre-paid return envelope. You will need to fill it out with information from your electric bill, sign it and return it to us to complete the registration procedure.
